Our outpatient CT imaging is a referral-only service. It is limited to stable, non-critical, non-urgent cases only. Cases will return to you for discussion of imaging results, further management, follow up care, and any client questions.
In order to ensure a smooth and efficient visit for your patient, please carefully follow the instructions below and let us know if you have any questions or concerns.
Please complete the online
referral form including client/patient information, brief medical history, and study request (please make sure to include any additional patient/client information that may be relevant to the case). As well as full medical records, recent radiology reports and images, and recent labs (CBC/Chem must be performed within 4 weeks of scheduled imaging).
Incomplete referral forms and needed details may cause a delay in scheduling.
All CT scans will be sent for a radiology review and the report will be sent to you once available (generally 2-4 business days). Images will be sent separately via email.
